If you do administration or procurement for a health care facility in the Northeast and are in need of face shields, please fill out the following form. If this is not your role, please find the appropriate person within your facility as having the proper contact information will help us streamline the fulfillment.
Your information will be sent to our by our Lead Organizer, and we will be directing face shields to facilities based on need. We will follow up with an email that contains more information. We will be delivering face shields as they are created and packaged on a continual basis, and we will reach out when we are able to deliver.
